Plication of the Diaphragm 101

Tuesday was a crazy day. Got a call in the morning saying plication of the Ryan’s diaphragm was a go for Wednesday (today!!).  I scrambled to find child care and was a bit bummed Ryan was having surgery at Thanksgiving, but felt if the doctors were advising it, then it was better to do it this week than wait a whole other week to do it. 

I geared myself up the entire day for Ryan to have his surgery this morning and be recovering on Thanksgiving day. The girls and I left the house around 4:30 p.m. to head up to my mom’s house so my sister (who just arrived home from college – yay!) could watch them. About 30 minutes into the trip, the general surgeon called to talk over the procedure. 
He brought up he was a little hesitant to do the surgery since Ryan had a positive blood culture (for bacteria) on Sunday and has only been on antibiotics for three days. In his experience, a patient being treated for an infection who goes into surgery typically ends up with a wound infection and has a harder recovery.  He said both he and the CICU docs were on the fence and he would only go forward if I was in a hurry to get it done. 
My response – why in the world would be push forward if it wasn’t in Ryan’s best interest??  So, we postponed the surgery until Monday morning.  
I know I promised to tell you guys what plication of the diaphragm is should we come to that decision so here is a brief description. Basically, the right side of Ryan’s diaphragm is paralyzed (a common side effect of open heart surgery). When you inhale, your diaphragm moves up and then on exhale it is supposed to move back down. Instead of shifting down on the exhale, Ryan’s right side stays up in the lung space possibly cramping that side and making it hard for him to inflate the right lung. 
With plication, the surgeon tacks down the diaphragm out of the lung space. He basically stitches it back onto itself giving it less slack. Over time, this stitch may work itself out or the diaphragm may stretch back out, but in that time, many people are able to gain strength and grow into a better breathing technique. That is the hope with Ryan. He has so many things going against him, we are hoping that by eliminating one of them he will be able to wean off oxygen. 
Please pray for Monday that Ryan will tolerate yet another surgery and that the results will be Ryan can wean off the oxygen.

    Leighann my husband and I prayed for Ryan this morning, and for all of you, but we had no idea that he was needing another surgery. I am glad it can wait until Monday. I hope you will enjoy Thanksgiving now. Try not to worry, but simply rest in the Lord and trust in Him.

    I will be praying for him and the surgery on Monday. I pray he can recover quickly and maybe even go home real soon. That would be so wonderful. And an answer to everybody’s prayers!

    We are praying for Ryan. How has he been ? Our baby Michelle had her right diaphragm plicated few days after her heart surgery performed at 6 days old. She is now 4 months since her gestational birth, a few days above 2 months corrected age. How was Ryan’s work of breathing with the plicated diaphragm ? We have always been concerned with our baby’s apparent work of breathing/ head bobbing after the surgeries. We would appreciate if you can help shed light on what to expect for babies with plicated right diaphragms. God bless.
